While none of the fields for the dam or dam's owner are required, the breeding will not be listed in the USCA database for the stud dog if the name of the dam, her dob, coat type and at least one registration number are not provided.
  
The owner of the stud dog, must provide their signature at the bottom of the form and then click the Submit button to save the information and their signature.  If you would like a copy of the completed form, you may use the Print Form button at the bottom of the form that will be displayed once the Submit button has been clicked.

There are 3 types of SZ registration number formats.
SZ: Dogs registered in the SV breed book
SZ-L: Dogs registered in the SV breed book as LSH
SZ-R: German Shepherds registered in the addendem register
All require a space after the letters and before the numbers.
SZ requires 7 numbers, SZ-L requires 3 or 4 numbers and SZ-R requires 8 numbers.
